Complete and print - registration form
New medical questionnaire for patients over 12 years old.

Please complete the registration form, and the questionnaire to register as a patient at Cromarty Medical Practice.
You need to print the registration form, take it to the reception desk at the practice and hand in the forms. You will be asked to provide relevant ID to one of the admin staff on duty.
Alternatively, come in to the surgery to collect the forms and hand them in once completed.
Once you have handed in your new registration form, please ask for an appointment with the health care assistant for a new patient check.
