If you’re staying in the area for more than 3 months you will have to join the practice list.
Once you are registered with the practice you are free to see any doctor you choose. Please note it is always best to try and see the same doctor throughout a period of treatment to ensure continuity of care.
If you are registering your baby then please bring along the form given to you by the Registrar of Births.
If possible, please bring with you 2 forms of identification – one photographic and the other with proof of your address. However, this is not essential and you will still be able to register without identification.
Patients can complete the permanent GP registration form online and then email it to the practice at nhsh.gp56538-admin@nhs.scot

There are some circumstances in which you may not be able to register with our practice, for example, if:
You’ll need to register with another GP practice in your area instead.
If you’re not staying in the area for more than 3 months, you may still be treated but as a Temporary Resident
In addition to completing the registration form, It would be helpful if you could fill in our new patient questionnaire. This is a document that allows us to gather some information in regards to your current, and past, medical history that may be beneficial for a clinician to be aware of if you have an appointment while we wait for the transfer of your medical notes from your previous practice. Please be aware that, in some circumstances, if you require medication or treatment urgently you may be asked to request a patient summary from your previous GP.

Please complete the questionnaire and return it along with the registration form either directly to the practice via reception or by emailing them to: nhsh.gp56538-admin@nhs.scot
You can leave the practice list at any time. This may be for a variety of reasons, such as moving away from the area. You do not have to tell us you are leaving, or the reason that you are leaving but it may be helpful to us to know.
In very rare circumstances we may have to remove a patient from the practice list. This would only occur when we feel that the trust that must be present between the patient and the doctors no longer exists.
Any abuse of, or violence towards any member of staff working within the practice, or other members of the public frequenting the practice, may lead to removal.
